1. To apply for a name change in an Attorney License, please submit to the Ministry of Justice the application form, the original Certificate of Attorney Qualification Examination issued by the Examination Yuan, and other documentation as listed on the application form.
2. Please be sure to fill in every field on the application form to protect your rights and interests.
3. Please provide a copy of the front and back of your identification card to prove your identity. Please do not use a driver's license, or other form of documentation for identification.
4. The original Certificate of Attorney Qualification Examination issued by the Examination Yuan. Please apply to the Examination Yuan for a name change first.
5. If you want to send your application by post, please send it to No. 130, Section 1, Chongqing South Road, Taipei City, Zip 100 (for recipient, please write the "Ministry of Justice").
6. Processing Time: It takes no more than 20 days (calendar days) to complete the process, starting from the time when the post is received by the mail room of the Ministry of Justice. Once completed, the license or a notification for pickup will be sent.

2. Mail application via registered mail.
